This prospective, randomized, parallel-group, open-label study with blinded endpoint evaluation assessed the effects of niacin extended release (ER) or ezetimibe on carotid intima-media thickness (CIMT) when both were combined with statin therapy.a
Subjects (N=208) had a mean age of 65 years and 80% were male. They had either known atherosclerotic coronary or vascular disease or coronary heart disease equivalent, 10-year Framingham risk score ≥20%, or coronary artery calcium score >200 for women or >400 for men; were receiving statin therapy for mean of 6±5 years (95% were receiving atorvastatin or simvastatin; mean dose was 42±25 mg); and had LDL-C ≤100 mg/dL and HDL-C ≤50 mg/dL (men) or ≤55 mg/dL (women).
Subjects received niacin ER up to 2,000 mg qd (increased from an initial dose of 500 mg at bedtime by 500 mg every other week to maximum tolerated dose [up to 2,000 mg])+ statin (n=97) or ezetimibe 10 mg qd + statin (n=111). The study’s primary endpoint was between-group difference in change in mean CIMT after 14 months.a
At 14 months, niacin therapy had a greater effect on change in mean and maximal CIMT vs ezetimibe. There was a significant between-group difference of P=0.003 for the repeated-measures analysis.
A significant reduction in mean CIMT was observed in the niacin group between 8 and 14 months (P=0.02). No significant net changes in CIMT were seen with ezetimibe. However, there was a significant inverse relationship between changes in LDL-C level and CIMT among ezetimibe-treated subjects (R=−0.31, P<0.001), such that a paradoxical increase in CIMT was seen among those with greater reductions in LDL-C. This inverse relationship was not observed among niacin-treated subjects (R=−0.01, P=0.92). There was a higher incidence of major adverse cardiovascular events among subjects in the ezetimibe group vs the niacin group (5% vs 1%; P=0.04 by the chi-square test).
