Patient Education
Communication between clinicians and patients is a critical component of any treatment and management regimen. Patient education handouts from CCMDweb.org are designed to supplement this important dialogue and reinforce important health messages for your patients with cardiovascular and metabolic diseases and conditions, including dyslipidemia, hypertension, diabetes, stroke, inflammation, and obesity.

These informative handouts cover myriad topics related to diagnosing, treating, and managing cardiometabolic diseases—facts about diseases, including coronary artery disease (CAD), peripheral arterial disease (PAD), hypertension, and diabetes; information on obesity and related topics such as weight control and appropriate dietary considerations; and much more.

These tools are designed to complement patient management. We encourage you to download and distribute these handouts to your patients to facilitate an open dialogue and enhanced understanding of their disease.
